It's the third and final Armageddon 2001 Superman Annual. Waverider's checked on Supes twice already and each time gotten a different result. Superman's not Monarch! Give it up, already! All kidding aside, Louise Simonson and Bryan Hitch produce the best of these Annuals, with beautiful art and an emotional story.

The true history: Though Lois and Clark did get married, they never had any (biological) children.

Altered history: At some point after the wedding, Lois becomes pregnant. Unfortunately, one kick from the Kryptonian baby and she's dead.
A devastated Superman leaves Earth for space where he is waylaid by aliens and saved by Maxima. Now, Maxima's tried to forcibly get Superman to be her husband before, but he's always spurned her advances. Still, he's from good genetic stock, so perhaps Max can get him to fall in love with her while nursing him back to health. Acting on her handmaiden Sazu's advice, she feigns weakness so that Supes will see a different side of her. Also, skimpy costumes.
By the time they reach her home planet of Almerac, Clark has warmed up to her and maybe she's fallen a little more for him. However, the Lords of Almerac are about to betroth her to De'cine, a really gross cyborg from an enemy empire. It would create an alliance and insure the progeny she owes her people. She turns him away, but Superman's not her in pocket yet, especially after she goes crazy with rage and whips a few prisoners. Still, the incident ends with her apologizing and Superman realizing maybe he could love this woman. But Earth and Lois remains on his mind, which distresses Maxima. Sazu takes it upon herself to help as if she were in an alien production of a Molière play, and manipulates De'cine into heading off to destroy Earth so that Clark could never return.
Meanwhile, Clark shows Maxima that you catch more flies with honey than vinegar, which she embraces. This makes him fly back to Earth to consider his options, say his goodbyes, whatever. Of course, he finds De'cine there about to destroy the planet. When Maxima finds out about Sazu's little plan, she flies off to help her man (throwing Sazu into a dungeon), and only with her help is Superman able to save Earth. De'cine's death will be avenged by his queen-mother, it seems, so Superman agrees to return to Almerac forever to insure Earth doesn't get caught in the crossfire. Almerac likes a good little war anyway. And so Kal-El and Maxima start on the task of making super-powered babies...
Books canceled as a result: While Superman's been off-Earth for extended periods of time, such a permanent change would probably mean cancellation.
These things happen: Lois hasn't yet become pregnant, except in the films (booo!), but it could still happen. Armageddon 2001 takes place 10 years after 1991 continuity, but 10 years have NOT passed in the last 20 from the character's point of view. Every time Superman's been touched by Waverider, he's gotten a premonition that's allowed him to cheat destiny. Perhaps it'll kick in when Lois becomes pregnant and he'll get her a gold kryptonite maternity trinket or something.
