I think you're right about those being eyes on Rhino's costume; it was more obvious on his original outfit because the eyes were more centered. Rhino actually did lose his costume in his second appearance (Amazing 43), when Spidey mixed some kind of acid into his web fluid to melt the costume off. I'm not sure when it became permanently grafted to his body, but Rhino's main motivation in the Deadly Foes of Spider-Man miniseries was to get enough money to remove the costume. The ironic thing is that once he got it off, he got bored and went to Justin Hammer to get a new outfit, though presumably that one was removable.
And there is a character called the Hippo, a Spidey enemy who first showed up in 2009's Dark Reign.
